Everything you need to register your boat or vehicle, request a boat slip, submit a project for approval, or reserve a common area — in one place. Forms can be submitted online through the Member Portal or by paper as described below.

Choose the option that works best for you. Online submission through the Member Portal is the fastest way to get a response. Paper forms are available for download below.

Event Request Form For members who would like to host an event open to the whole membership — a picnic, holiday activity, fishing derby, or similar. Include your proposed budget for Board review. This is separate from the Common Area Party Request Form below, which covers private gatherings.

Boat Registration Form Required for all boats used on Countryside Lake. Complete and return to receive your CLA decals and registration number.

Pier Waiver — Waiver of Claims & Hold Harmless Agreement Required annually for all pontoon boat pier holders. Must be signed and returned with your pier fee payment by February 1. Pier space is forfeited if the fee and signed waiver are not received by March 1.

Project Approval Request Form Required for any change to the exterior of your property — things such as new construction, additions, garages and sheds, fences, pools, solar, docks, dredging, and Shore Area work. Like-for-like repair or replacement does not require approval. Submit at least 30 days before your intended start date.

Common Area Party Request Form Required for gatherings of 15 or more guests using a Common Area (beach or tennis courts). Submit with your $100 non-refundable fee prior to the event date.

Member Information Form Used to provide or update your contact details, vehicle information, and occupant information with the Association. Required for new members and when information changes.

Unit Owner Consent to Electronic Delivery of Notice Authorize the Association to deliver official notices to you by email rather than US mail. Optional but recommended for faster communication.

Member/Lessee Privileges Agreement Paper form only — not available through the portal. Required when leasing your CLA property. Must be signed by both member and lessee and returned within 10 days of signing the lease.

Architectural & Project Approvals

What requires approval Any change to the exterior of your property requires the Board's written approval before you begin — anything that alters its appearance, footprint, or the structures on your lot. Association-wide, this includes things such as new construction and additions, other buildings such as garages and sheds, fences, swimming pools, solar energy systems, boat docks, dredging, and any landscaping or construction in the Shore Area. Routine maintenance and like-for-like repair or replacement, using the same materials, colors, and dimensions, do not require approval. If you are unsure which side of that line your project falls on, ask us before you start.

Timing and penalties Plans must be submitted at least 30 days before your intended start date (30–60 days for Shore Area and shoreline projects). Work may not begin until written approval has been received. Proceeding without approval carries a $1,000 fine and may require reversal of completed work at the member's expense.

Your subdivision may have its own rules Many other exterior projects are governed by your subdivision's covenants rather than the CLA, and requirements vary from one subdivision to the next. If you're unsure whether your project needs approval — or whose approval you need — the parcel map below shows which association each property belongs to. When in doubt, contact us before you begin. A quick inquiry can prevent a costly problem later.
